Issuance of Medical Services Card to former Provident Fund Scheme members

 
  Upon cancellation of enrollment, a member shall maintain his/her right to medical services, as entitled while in service, when:  
 
1) At the date of cancellation of enrollment, the member reaches 50 years of age and his/her contribution time is not less than 25 years;
2) The contribution time is not less than 15 years and the member:
2.1) has reached the age of 65, except when there is a different age limit otherwise prescribed by other legislations; or
2.2) has reached the upper limit of sick leaves; or
2.3) has been declared totally and permanently incapable for work, or died.
3) Being declared totally and permanently incapable for work, or died, as a result of job-related accident or sickness, humanitarian act or dedication to the community.

The spouse of the member referred above, as well as the descendants and ascendants of the member and of his/her spouse are also entitled to have access to medical services as prescribed by law.
 
  In order to have access to medical services, monthly contribution, calculated based on the monthly salary or wages of the member the day before cancellation of enrollment in the Scheme, should be made to the Health Bureau.

  Procedure and documents required  
  The member shall submit the followings:  
 
1) Written form (Model no.RP-9A) provided by the Pension Fund and can be downloaded from "Forms Download";
2) Copy of identification document of the member and the eligible family members;
3) One recent color photo of 1.5 inch of the member and each eligible family member, with the name written on the back;
4) Document proving the relationship between the member and the family members (e.g. marriage certificate, birth certificate etc.);
5) Document proving the eligibility of the family member(s):
5.1) Spouse:
a) In the case of being employed:
It is necessary to submit a declaration issued by his/her employer's entity stating that he/she is not a beneficiary of any health care scheme;
b) In the case of being self-employed:
It is necessary to submit a documentary proof (e.g. payment advice of contributions of the arbitrary system issued by the Social Security Fund for the previous quarter, or a notification of evaluation of revenue for the professional tax (M-16) issued by the Financial Services Bureau);
c) In the case of being unemployed:
It is necessary to submit a documentary proof (e.g. a proof of no contribution payment record at the Social Security Fund, or payment advice of contributions of the arbitrary system issued by the Social Security Fund for the previous quarter.)
5.2) Descendants:
a) In the case of a descendant under the age of 18, only a document proving the relationship is required;
b) In the case of a descendant between 18-24 years old, a certificate of enrollment is required;
c) There is no age limit for descendants mentioned below:
1) In the case of a descendant who is suffering from a long-term disease or is physically or mentally incapable, but is still exercising a certain activity, a medical certificate is required;
2) In the case of a descendant being sent to a re-educational institution, a relevant documentary proof is required;
3) In the case of a descendant suffering from a long-term disease or being physically or mentally incapable of work, a medical certificate is required.
If a descendant reaches the upper age limit for receiving the family subsidy during a school year of study, his/her right to medical services can be maintained until the end of that school year.
For conditions mentioned in b) and c-1), the total annual income, including returns, rentals, pensions or other revenues, of each descendant cannot exceed an amount equivalent to the index of 600 in the salary scale for the Public Services. (In the case of receiving work income, it is necessary to submit a declaration issued by his/her employer's entity stating that he/she is not a beneficiary of any health care scheme.)
5.3) Ascendants:
 
a) In the case of being employed:
A proof of income is required. The total annual income, including returns, rentals, pensions or other revenues, of each member of a married couple / of non-married ascendants / of a married couple that has in fact separated for more than 2 years, cannot exceed an amount equivalent to the index of 600 in the salary scale for the Public Services, and it is necessary to submit a declaration issued by his/her employer's entity stating that he/she is not a beneficiary of any health care scheme;
b) In the case of being unemployed:
It is necessary to submit a documentary proof (e.g. a proof of no contribution payment record at the Social Security Fund, or payment advice of contributions of the arbitrary system issued by the Social Security Fund for the previous quarter.)
 
     
  Place of submission  
  The Pension Fund, Centro de Serviços da RAEM, Centro de Serviços da RAEM das Ilhas, the Central District Public Services Centre or the Islands District Public Services Centre – Seac Pai Van Station.

  Time needed  
  The medical services card shall be approved and sent to the Health Bureau, within 5 working days counted from the following day after submission of all necessary documents (Performance Pledge), and after being notified by the Health Bureau, the beneficiary can go to the Bureau to pay the contributions for collecting the medical card.

  Duty of notification  
Holders of medical services cards have to fulfill their duty of notification. Under foreseeable circumstances, they should inform the Pension Fund, in advance, of the fact that the conditions for maintaining their right to medical services can no longer be met, or they should notify the Pension Fund within 15 days after the occurrence of that fact. Meanwhile, the medical services card must be returned to the Pension Fund. Relevant supporting documents leading to disqualification must also be submitted, such as graduation certificate/last school year transcripts or any other study certificate of the adult descendant, death certificate.

   (1) Application procedure and documents required (In case of death of the member)  
  In case of death of the member, if his/her eligible heirs want to maintain the right to medical services, they shall approach personally the places of submission referred above, to submit the followings:  
 
1) Written form (Model no.RP-9B) provided by the Pension Fund and can be downloaded from "Forms Download";
2) Copy of identification document of the eligible family members;

5) Document proving the eligibility of the family members:

It is necessary to submit a document which can prove that the applicant is the last spouse of the deceased member (e.g. death certificate/marriage certificate/birth certificate etc.).
5.2) Descendants:

b) In the case of a descendant between 18-24 years old, a certificate of enrollment is required;
c) There is no age limit for descendants mentioned below:

   (2) Electronic services of the medical services card  
  If the beneficiary, in conditions of access to health care, wants to use the “Electronic services of the medical services card” provided by the Health Bureau, he/she can submit the “Data transfer consent form” together with the application for the medical services card.
